• MYSL MSPSP and LEAGUE AFFILIATION forms:
• MYSL LEAGUE AFFILIATION Regulations and Checklist
  
• MYSL LEAGUE AFFILIATION Form - ( New for Fall 2010/Spring 2011) for all Leagues and Clubs due at every Registration.
  
• MSPSP AFFILIATION FORM - for all teams due at annual application to MSPSP.
• MYSL INSURANCE AFFIRMATION form (new for Fall 2010/SPRING 2011) All Leagues/Clubs must submit this form.

• MYSL FALL 2010 Early Pass card Request Form
• MYSL Treasurer's Registration FALL 2010  (Please Note this may take a minute to open) - Must be submitted by every League/Club at registration
  MYSL TEAM AFFILIATION FORM (new FALL 2010) ALL TEAMS must turn in to the Commissioner at registration.
• MYSL TEAM REGISTRATIONFORM (new FALL 2010) ALL TEAMS must turn in to the Registrar at registration.
